For Doctors

Referrals must be FAXED to our office (905-820-3352)

This must include:

• Complete patient demographics
• Past medical / surgical history
• Medications
• Relevant up-to-date results (imaging, lab reports)
• Your name, contact information and referring physician 6-digit billing number

Referrals will be triaged by the appropriate surgeon and given back to our office staff who will contact your patient directly with an appointment.

Please note that we DO NOT see patients regarding the following:

• Pediatric patients (no one under the age of 18 years)
• Skin lesions
• Hidradenitis
• Head and neck (including thyroid problems)
• Circumcision
• Ingrown toenails
• Follow-up for surgery done by another surgeon/centre or out of country
• Weight loss surgery or new Lap Band referrals

Referrals sent to us regarding any of the above will be returned to your office to be re-directed to the appropriate physician.

Urgent Referrals

• May be addressed to “first available surgeon”
• Appropriate referrals will be triaged and booked with the first available surgeon
• If we cannot accommodate the referral on an urgent basis, we will notify your office

Endoscopic Procedures

Endoscopic procedures are booked through our office to be performed in the Endoscopy Clinic at The Credit Valley Hospital.

• Missing information will result in a delay in your patient being seen

Referrals will be triaged, and your patient will be contacted by a hospital staff member to book an appointment.

These appointments will take place in the clinic at The Credit Valley Hospital, NOT in our private office.
